| ☐ | Cisco ACI pre-upgrade checks and configurations completed and upgrade impacting faults identified and remediated. |
| ☐ | Notify key business stakeholders of change >7 days in advance. E.g. Change management and Application teams. |
| ☐ | Detailed sequence of events implementation and regression steps documented. |
| 2. Implementation Steps | |

| ☐ | All hardware to be replaced is pre-staged and ready for onboarding. |
| ☐ | Key business applications and network services have been isolated. |
| ☐ | Pre-change baseline checks have been completed and documented, and network and security appliances have been backed up to a remote location. |
| ☐ | New software versions have been uploaded and MD5 checksum validated for authenticity. |
| ☐ | Perform software upgrade based on business requirements and the mapped upgrade groups. |
| ☐ | Replace end-of-life hardware. |
| ☐ | Perform post-upgrade testing to ensure key configurations and applications are performing as expected in line with the pre-change baseline checks. |
| ☐ | Notify key stakeholders of change completion. |
| 3. Post-Change Actions | |

| ☐ | Update documentation to reflect new software and hardware details. |
| ☐ | Ensure operations monitoring dashboards are updated. |
